This year and moving forward, we would like to make wellness an important part of our worksite culture! Research has shown that promoting health and wellness at work can improve morale, decrease stress & most importantly lead to a happier and healthier work environment.

We are excited to announce our Annual Physical Exam Certification Promotion and look forward to your support and participation. If you receive your annual physical exam between October 1, 2019 and September 30, 2020 and are benefit eligible, you will receive a $100 Target gift card within 45 days! Please note that the deadline to submit this form to receive the reward is October 30th, 2020.

We also would like to highlight the wellness resources that are available at no cost through Aetna, if you are an Aetna member. These programs will provide you with tools to help you lead a healthier lifestyle. Please review this site to learn about the programs that are offered to you and your family.
